This is an optional undertaking to sub-specialize in a particular field of medicine, which can take 1-3 years.
Again, you need to apply and compete for seats. As a fellow, the residents report to you as if you were a staff member, but you still review cases with the staff. People who take fellowships often end up working in large centres, as smaller centres do not generally have a need for such sub-specialized care. Each area of medicine that you can apply for through CaRMS has a variety of fellowships available, however not every university setting offers every fellowship.
